Foreigner Physical Examination in China: Work and Study Guide

Do foreign workers need a physical examination for China?

For work in China lasting more than 90 days, a health document is part of the work-permit materials described in current Beijing official guidance. Acceptable routes listed there include a verification certificate or health certificate issued by a Chinese inspection and quarantine institution, or an examination certificate issued by an overseas medical institution recognized by the Chinese inspection and quarantine authorities. The document must have been issued within six months.

The same Beijing guidance says that before entry an employer may use the health-commitment route, with the applicant completing the verification or health certificate at a Chinese inspection and quarantine institution after entry. This is an official Beijing service rule, not proof that every local authority or every employer will use that route in every case.

Before arranging an overseas examination, ask the employer or authorized agent to confirm:

  1. the city and authority handling the work-permit application;
  2. whether the current application stage accepts a health commitment;
  3. whether an overseas institution must appear on a recognized list;
  4. which original reports, signatures, stamps and translations are required;
  5. which International Travel Healthcare Center will handle the post-arrival examination or verification;
  6. the deadline for the work permit and residence application.

Do international students need a physical examination?

Students should follow the instructions from their admitting university and the authority handling their study residence permit. Beijing Normal University's current guidance says that students entering with an X1 visa need a physical examination for conversion to a study residence permit. It says students may complete an examination abroad and submit the original form and reports for verification in Beijing, or complete the examination after arriving in China. If the overseas record does not pass verification, another examination may be required.

The same university guidance says that X2 students generally do not need the examination unless the local Chinese embassy or consulate specifically requires it. That is university guidance for its students, so an applicant elsewhere should not use it instead of instructions from their own institution.

Ask the university's international-student office to confirm:

  • whether your visa and planned length of study require a study residence permit;
  • whether you should complete the examination before travel or after arrival;
  • the local designated examination or verification center;
  • the originals and school documents you must bring;
  • the collection time and the residence-permit filing deadline.

Can an overseas medical examination be used in China?

Potentially, but acceptance is not automatic. Current Beijing work-permit guidance says an overseas certificate must come from an overseas medical institution recognized by the Chinese inspection and quarantine authorities, and it points applicants to the relevant Chinese embassy or consulate for the recognized-institution list. The document must be within six months.

Shanghai's official health-check guide tells applicants who have an overseas report completed within the previous six months at an embassy-designated hospital to bring the originals, including laboratory results and the original X-ray film, for reference. Beijing Normal University's student guidance similarly requires the original physical-examination form and original supporting reports for verification.

Before using an overseas provider, obtain written confirmation of all of the following:

  • the exact form version;
  • whether the provider is recognized for your destination and application;
  • whether every section must be completed;
  • where the doctor's signature and institutional stamp must appear;
  • which original laboratory, ECG or imaging materials must be attached;
  • whether a Chinese translation is needed;
  • the permitted issue date and validity period.

A clinic saying that it performs “China visa medicals” is not, by itself, evidence that the receiving authority will accept its report.

Where is the examination or verification done after arrival?

The official route is normally an International Travel Healthcare Center connected to China's customs system, not simply any convenient hospital or private check-up clinic. The relevant center and procedure depend on where the work, study or residence application is handled.

For example, Shanghai's official guide directs qualifying applicants to the Shanghai International Travel Healthcare Center and gives separate supporting-document examples for work, study, accompanying-family and other residence purposes. It also provides a current appointment route, address and collection process. These details are useful for Shanghai only and can change.

Use the city pages for Beijing, Shanghai, Guangzhou and Shenzhen to orient yourself, then confirm the exact center through the local government, customs health center, employer or university. Do not book a hospital merely because it appears in the China hospital directory. A profile such as Peking Union Medical College Hospital describes a clinical hospital; it does not mean that the hospital is the designated center for immigration-related examination or report verification.

What should you prepare for the appointment?

Requirements differ, but the official Shanghai guide provides a useful example of the categories commonly requested. Prepare a folder containing:

  • your original passport;
  • copies of the passport photo page, visa or entry page, and any information-change page;
  • recent matching identity photos in the size specified by the local center;
  • the center's completed application and consent forms;
  • work-permit documents, employer documents, student ID or admission notice, depending on the purpose;
  • every original overseas examination document, laboratory report and imaging item if seeking verification;
  • the appointment confirmation and a reliable collection plan;
  • any additional document specified by the center, employer, university or local authority.

Shanghai's current guide instructs applicants to avoid food for at least five hours before the examination while allowing water. Do not assume this instruction is identical everywhere: follow the preparation notice issued by the center where you actually booked, especially if you have a medical condition, are pregnant, take medication or cannot safely fast. Contact the center or your clinician for individual instructions rather than changing medication on the basis of a general article.

How long will the certificate take?

There is no single nationwide collection time that ChinaMedGuide can promise. Shanghai's official guide, updated in July 2026, says the health certificate can be collected three working days after the examination, or sent by courier under the center's process. Other centers may use different appointment capacity, verification steps, re-examination procedures and collection times.

Build extra time into the plan. A missing original, incomplete overseas report, inconsistent passport name, public holiday, full appointment calendar or request for re-examination can delay the certificate. Do not schedule onward travel on the assumption that the result will be issued the same day.
